By all accounts, this love affair with toast began in San Francisco in a hole-in-the-wall cafe called Trouble Coffee and Coconut Club run by a woman with schizo-affective disorder, a double whammy illness with symptoms of schizophrenia and bipolarity - which often led to 'trouble'.
